MVR OWNERSHIP AGREEMENT

breed this horse, and that if I decide to transfer my ownership rights of

________________________, I will not dispose of this animal at

auction, transfer ownership to a known horse trader/ broker or abuser, or turn over custody to anyone that would allow this horse to be taken to slaughter. I understand that by signing this agreement I am stating that I will try, to the best of my ability, to see that this animal is transferred to a humane and caring new home. I further agree that I will file the appropriate paperwork with Mountain View Rescue, Inc. within thirty (30) days showing any such transfer, along with a copy of the new owner’s agreement, including their name, physical address, and phone number.

 In the event of my death this horse is to be returned to Mountain View Rescue, Inc unless other arraignments are made with my heirs.

In addition, if this animal should die while in my custody, I will report such a death to Mountain View Rescue, Inc with a copy of the Veterinarian report of cause of death within thirty (30) days of passing.
